Create a Forum link not working...

Seems like a simple thing, but I don't know what I am doing wrong!  I am the manager of the group.  When I click on Create A New Forum, and I complete the fields...Name, description, and hit Submit, it thinks and then just clears all the fields and I am at the beginning again.  No error message, no forum created.  What on earth am I doing wrong?  Is it because I am not entering anything into the Email field?  what do I even enter there?  I have tried my name, and the same thing happens.  Can anyone help?
